I handle all aspects of registration, withdrawals, and records. My goal is to support you in making a smooth transition to Peachtree Middle School, ensuring excellent service for all students, staff, and parents. Please note that my office is 100% virtual.

Registration Process:
Complete the Registration Application (Please allow 24-48 hours for your application to be processed):
Receive Status Update:
- After your application is submitted, you’ll receive an update via Infinite Campus once it’s approved.
Finalize Enrollment:
- Once approved, contact the registrar to enroll your child at Peachtree Middle School.

Forms and Examples of Acceptable Documents
Proof of Residency Accepted:
Current Mortgage Statement or Lease:
- Must show parent/guardian name.
One (1) Current Utility Bill:
- Acceptable utilities: Electric, Gas, or Water.
- Must show parent/guardian name.
- Note: Telephone/cell phone bills, cable bills, insurance, or bank statements are NOT accepted.
Parent/Guardian Current Official Picture Identification (MUST BE CURRENT)

Health Authorization Forms:
Objection to the Use of Social Security Number:
Notarized Affidavit of Residence Form:
Purpose: This form is for students living with a parent or legal guardian at another adult's residence within the DeKalb County School District boundaries.
Validity: The affidavit is valid only for the current school year and must be submitted annually or when moving into a new attendance area.
