Registration Requirements

DeKalb County New Student Registration Requirements

*Only a parent or legal guardian of a student may register the student for school.

Please bring the following information with you to register your student:
Proof of Residency: copy of lease or contract and a utility bill or a statement of legal   residency to be completed on form which school will provide (Affidavit)

Current Health Records: Required by Georgia Law (transfer to Georgia Form 3231 and Form 3300)
>Form 3231: complete immunization records; includes 2 MMR 
            (Measle, Mumps, Rubella), Varicella (Chicken Pox), 
            and Hepatitis (Recombivax 11-15 years)
 
>Form 3300: Hearing, Vision, Dental, and Nutritional screenings
 
*A physician or the local health department can provide all necessary forms.

Student withdrawal form (from former school with school’s complete address)

Final grades, standardized test scores, with other documentation for Gifted, Special Education, 504, and/or SST

Copy of Student’s Birth Certificate

Copy of Student’s Social Security Card

Copy of Parent’s or Legal Guardian’s Driver’s License

Legal Guardianship/Custody paperwork (if applicable); Documentation of guardianship or temporary guardianship can be obtained through DeKalb County Probate Court



STUDENTS NEW TO DEKALB WHO ARE BORN OUTSIDE THE UNITED STATES

AND/OR WHOSE PRIMARY LANGUAGE IS OTHER THAN ENGLISH MUST

COMPLETE REGISTRATION AT THE DEKALB INTERNATIONAL WELCOME

CENTER LOCATED AT 1701 MOUNTAIN INDUSTRIAL ROAD, STONE MOUNTAIN
