Greetings Parents/Guardians,
The head counselor, Ms. Havens, will facilitate classroom guidance lessons designed to teach students how to detect warning signs associated with depression, suicidal thoughts and self-harm. Students will also learn when it is time to A.C.T. (Acknowledge the warning signs, show your friend that you Care, and TELL a trusted adult). Please see all attachments for additional information about this nationally recognized program.
If you do NOT wish for your child to participate in SOS Program in school, sign the attached permission form and return to the PCMS Counseling Department to the attention of the head counselor, Ms. Havens. If we do not hear from you, we will assume your child has permission to participate in this program.

La consejera principal, la Sra. Havens, facilitará lecciones interactivas de orientación en el aula diseñadas para enseñar a los estudiantes a detectar signos de advertencia asociados con la depresión, pensamientos suicidas y autolesiones. Los estudiantes también aprenderán cómo y cuándo es el momento de ACTUAR (Reconozca las señales de advertencia, muestre a su amigo que le importa y le DIGA a un adulto de confianza).
Estamos incluyendo una copia del boletín para los padres y la lista de recursos de referencia de salud mental para que tenga información sobre la depresión y sus riesgos.
Si NO desea que su hijo(a) participe en el programa SOS para escuelas secundarias (SOS Middle School program), por favor llene esta forma y envíela a Peachtree Charter Middle School con atención a Sra. Havens. Si no recibimos su respuesta, asumiremos que su hijo tiene permiso de participar en este programa.
